How Does Thyroid Radiofrequency Work?

Limitation in mass size: This technique is often effective for nodules smaller than 4 cm. For larger nodules, surgery is usually a more suitable option.
Number of nodules: The highest success rate of RF is observed in the simultaneous treatment of one to three nodules. If there are multiple nodules in different thyroid lobes, the effectiveness of this method decreases.
Recurrence: In some cases, the thyroid nodule may grow back after treatment. In this case, the patient may need repeated RF sessions.
Strong dependence on the skill of the doctor: The final result and the prevention of damage to the nerves in the neck depend entirely on the experience of the radiologist.
Side Effects of Thyroid RF

Radiofrequency is not considered safe during pregnancy because of the possibility of unpleasant side effects for the fetus.

Proximity to vital structures
If the thyroid nodule is located in the vicinity of sensitive structures such as the esophagus or major blood vessels, the use of RF is not recommended due to the possibility of unwanted heat transfer to these tissues and causing irreversible complications.
Proximity to the vocal cord nerve:
If the location of the nodule is too close to the nerves controlling the vocal cords, there is a risk of voice damage and RF is not performed.
Follicular Adenoma Thyroid Cancer:
Radiofrequency is not recommended for follicular thyroid tumors because benign and malignant follicular cells are similar. When a follicular cell breaks through the sheath, it is diagnosed as malignant. This means that the entire mass must first be surgically removed and, after a thorough examination, its benignity or malignancy can be determined. In fact, burning the tissue eliminates the opportunity for an accurate diagnosis of pathology.
Conditions of Thyroid RF in Follicular Cancer
One major limitation of RFA is follicular thyroid nodules. Neither FNA nor RFA can definitively differentiate benign from malignant follicular adenomas. Diagnosis requires complete surgical removal and microscopic examination. Therefore, RFA in these cases should only be considered with a multidisciplinary medical team.
